Eligible applicants should obtain an application packet from Grants.gov and ensure that:

  • All packet items are labeled correctly
  • Packet is properly formatted
  • Proposal should not exceed 45 pages

Each proposal should be submitted electronically and include:

  • SF424 Application Form
  • Budget
  • Budget Justification
  • Indirect Cost Rate Agreement
  • Project Description
  • Current Budget Progress
  • New Period Budget, Objectives, and Activities

The remaining balance proposal should be consistent with the previously awarded proposal and be realistic. The proposal must be complete and full detailed plans for the remaining funds should be included within the proposal. This program allows rewards to be extended for up to three years but must:

  • Express a bona fide need for the unused balance
  • Include a financial report for the remaining balance
  • Include activities proposal with a budget and narrative justification

Activities that fall into the following activities may qualify for funding extension under this opportunity:

  • Strategic Information (HVSI)
  • Laboratory Efforts
  • Care and Support (HBHC)

All proposal must be submitted in standard formatting before the deadline. Incomplete or late applications will not be considered for extended funding.
