The Small Rural Hospital Improvement Program (SHIP) was established to provide rural hospitals funds for:
- Equipment
- Training
- Becoming accountable care organizations
- Create shared savings programs
- Purchase health information technology, equipment, or training
- Payment bundling
Eligible participants are excluded to small rural hospitals with 49 beds or less; these hospitals must be:
- Non-federal, short-term general acute care facilities
- Located in a rural area
- Can be faith-based
Awarded grantees are required to manage and monitor the approved project and ensure operations adhere to the statutes, regulations, and grant terms and conditions. The grantee is required to:
- Ensure funds are spent on approved project activities
- Ensure funds are not used in violation of agreed upon terms
- Submit completed reports to the grantor in a timely manner.
- Ensure an adequate accounting system is implemented and maintained for a minimum of three years after the completion of the project
Average award per hospital is $9,000; fund may be used for activities including but not excluded to:
- Value-based purchasing
- Accountable care organizations
- Shared Savings
- Payment Bundling/PPS

- Budget
- Financial statement or audit signed
No late applications will be accepted and all application materials must be typed. Applications may be emailed or mailed to the Texas Department of Agriculture.
